If you and your family are currently enrolled in District 186's health plan, you are eligible for the benefits provided by Memorial Choice.

As an employer of choice in the community, District 186 is proud to offer Memorial Choice at no cost to you.  This service supplements your insurance plan by offering additional benefits that can help you live a healthier life and lower your healthcare costs.

How do I enroll in Memorial Choice?

If you are enrolled in District 186's health care plan, you should  receive a Memorial Choice member card in the mail by June 1.  If you do not receive a card by that date, please contact Mitzy Widebeck at mitzyh@sps186.org.

How do I access my Memorial Choice benefits?

To access your benefits - including $0 co-pay for select primary care and ExpressCare services - you must present your Memorial Choice member card along with your insurance card at every visit to a Memorial clinic.

How do I transfer my care to Memorial Physician Services?

If you do not currently see a physician at Memorial Physician Services (MPS), you are not required to make a change.  However, in order to access the full range of services available through Memorial Choice, you must transfer your care to MPS.  Call the Nurse Concierge line at 217-862-0NOW to make an appointment with an MPS provider and begin transferring your care.

How do I make an appointment with a Memorial provider?

The Nurse Concierge is available 7 a.m. - 7 p.m. Monday through Friday and 8 a.m. - 5 p.m. Saturday to help you schedule an appointment.

How do I get more help with my Memorial Choice benefits?
