Rates effective 6/1/18.

Blue Cross Blue Shield Health Insurance

New employees - enrollment must be within 30 days of the date of hire. Health benefits will begin on the effective date of hire.

Current employees - open enrollment will begin mid-April. Plan year begins June 1.

Current employees - you may enroll any time during the plan year if you have had an involuntary loss of coverage. You must enroll within 30 days of the loss of coverage.

6/1/17 - CVS Pharmacy No Longer Provider

Please be aware that effective June 1, CVS pharmacy will no longer be a provider for the District’s RX program.  This decision was agreed to by the insurance committee as a cost cutting move.

There is a big difference between a voluntary and involuntary loss of
coverage. It determines whether a life event qualifies as one that will allow an
employee to enroll in or make changes to existing insurance coverage.

   • An employee will only be able to enroll due to a Qualifying Life
    Event (QLE) when the loss of coverage is involuntary.
    • If other health coverage is available to an employee or their dependent
    and they decline it, the change is considered voluntary, and this
    does not count as a QLE.

These forms must be returned when applying for new coverage or adding a dependent to your coverage within 30 days of the date of the qualifying event.

Model Notices Previously Expired on April 30, 2017
The U.S. Department of Labor’s Employee Benefits Security Administration (EBSA) has extended the effective date of its model Health Insurance Exchange Notices through May 31, 2017. Previously, these model notices expired on April 30, 2017. No other changes have been made to these notices.

YOUR ELIGIBILITY AND EFFECTIVE DATE

If you enrolled during annual open enrollment, your coverage is effective June 1, 2017. If you are a newly eligible employee, you must enroll within 30 days of your employment date. Your coverage begins on your effective date of hire. Claims incurred prior to your effective date of coverage or after your plan termination date are not eligible for reimbursement.

OTHER PROGRAMS OFFERED BY BCBS

Blue365 is a discount program available to BCBSIL members. This is NOT insurance. Some of the services offered through Blue365 may be covered under your health plan. Please refer to your benefit booklet or call the Customer Service number on the back
of your ID card for specific benefit information under your health plan. Use of Blue365 does not affect your premium, nor do costs of Blue365’s services or products count toward any maximums and/or plan deductibles.

The Blue365 Eyemed discount will only apply to the products/services not already covered by vision insurance.
