Must be signed by the Payroll Dept. prior to taking the form to the YMCA to enroll.

Forms must be returned to the Payroll Dept. to have payroll deductions set up. Payroll deductions for health club membership fees are available only to full time employees.

TO CANCEL MEMBERSHIP, YOU MUST NOTIFY THE PAYROLL DEPT. IN WRITING TO STOP YOUR DEDUCTION. Emails are acceptable.

Membership fees cannot be stopped in the middle of the month.
