Registration for the 2012-2013 school year will take place at the Capital College Preparatory Academy, 1101 S. 15th Street, on Monday, August 6 from 11:00 a.m. to 6:00 p.m. and on Tuesday, August 7 from 8:00 a.m. to 2:00 p.m. If your child attended Springfield Public Schools last year you will need to bring the following information:

• Proof of residency
• Immunization records (incoming sixth grade students only)
• Proof of current physical examination (incoming sixth grade students only)
• Proof of current dental examination (incoming sixth grade students only)
• Free and reduced lunch application (if applicable)

Students who did not attend Springfield Public Schools last year will also need to provide a birth certificate. Additional information and registration forms are available on the District’s webpage.
