Uniform Vouchers

Call 535-3120 for appointment at Sangamon County Community Resources located at 2833 S. Grand Ave. East. This location is the former Cub Food store.
*Vouchers will be disseminated to parents of incoming Sangamon County students who attend school where uniforms are required to.
*Vouchers may only be redeemed at Luers at 1520 S. MacArthur. No cash back.
*Vouchers must be used within one week of issuance.
*Up to 4 uniform vouchers per eligible family (one per eligible child in family). unless approval granted. Appointments for students attending other schools must be made later.

DOCUMENTATION REQUIRED at Appointment:
1. A photo id for the head of household.
2. Proof of student enrollment at a uniform school.
3. Social Security cards for all household members.
4. Family income information for the 90 days prior to the appointment.
5. Any medical cards for household members.
6. If no income, must bring a denial letter from any of the following:
Social Security Benefits, Unemployment benefits, Workman’s Compensation Benefits or
or a medical card (front and back with client’s name on it).
7. DHS letter with proof of application for food stamps or cash/medical benefits.
