Physicals and Immunizations Due October 1 as Required by State Law

DUE OCTOBER 1,2020 as REQUIRED BY STATE LAW (Even while our students are on Remote Learning)

Attention Families still needing to get their child's SCHOOL PHYSICAL EXAM & REQUIRED IMMUNIZATIONS!

Please make an appointment for your child to get their school physical exam and required immunizations today. Most physicians’ offices are currently scheduling appointments now.

· All Pre-K, Kindergarten, 6th grade, 9th grade students and students new to Illinois schools must provide a current physical exam and immunization record. Students entering from another Illinois school must provide his/her most recent school physical and immunization record.

· All 6th grade students must show proof of having received a Tdap vaccine, 2 doses of varicella and three doses of Hepatitis B vaccine at approved intervals. One dose of the meningococcal conjugate vaccine must be received on or after the 11th birthday.

· All 12th grade students must provide proof of having received a dose of meningococcal conjugate vaccine on or after their 16th birthday.

All physical exams and immunization records must be received in the school office by October 1st to avoid exclusion from school!
