Health Services

All students entering a pre-k program, kindergarten, 6th grade, 9th grade and those new to Springfield Public Schools must provide proof of a physical exam completed by a physician, nurse practitioner or physician's assistant by the first day of school.  An updated immunization record is also required.  Students failing to provide all mandatory health information will be excluded on the  10th day of school and will not be allowed to return until all required records are received in the school office.

If you need assistance locating a physician, SIU School of Medicine at (217)545-8000 or Central Counties Health Center at (217) 788-2300 is available to help meet your child's health care needs.

Children entering kindergarten, 2nd, 6th and 9th grade are required to have a dental exam.  Students entering kindergarten and those new to Illinois schools are also required to have a vision exam.

Please contact the nurse at your child's school for questions.
