Please note the following information regarding registration and school physicals and immunization clinics for the 2021-22 school year.

If you need help registering in person, schools will be open today from 11 a.m. to 6 p.m. and tomorrow from 8 a.m. to 2 p.m. to assist you.
School Physicals and Immunization Clinic Information
Please note:
- All Pre-K, Kindergarten, 6th grade, 9thgrade students and students new to Illinois schools must provide a current physical exam and immunization record. Students entering from another Illinois school must provide his/her most recent school physical and immunization record.
- All 12thgrade students must provide proof of having received a dose of meningococcal conjugate vaccine on or after their 16th birthday.
- The Keep Kids In School Clinic will be held at Southeast High School tomorrow, Tuesday, August 10, 2021 from 8:30 AM - 1 PM everyone must be signed in by 12 NOON.
- Another Keep Kids In School Clinic will be held Saturday, August 14 from 8:30 AM - 12:30 PM at Springfield High School everyone must be signed in by 11:30 AM.
- It’s very important to note that all students must be signed in an hour prior to the closing time for each clinic session.
- All Pre-K, Kindergarten, 6th grade, 9thgrade students and students new to Illinois schools must provide a current physical exam and immunization record. Students entering from another Illinois school must provide his/her most recent school physical and immunization record.
- All 6thgrade students must show proof of having received all required vaccines including a Tdap vaccine, 2 doses of varicella and 3 doses of Hepatitis B vaccine at approved intervals. One dose of meningococcal conjugate vaccine must be received on or after the 11th
- All 12thgrade students must provide proof of having received a dose of meningococcal conjugate vaccine on or after their 16th
- Kindergarten, 2nd, 6thand 9thgrade students are asked to provide proof of a dental exam.
- Kindergarten students and students new to Illinois schools need a vision exam.
Physical exam and immunization information are due the first day of the 2021-2022 school year. Students failing to provide required health information will be excluded from school on the 10thday.
