Registration for the 2016-17 school year is this week. All elementary school students must register. Elementary school registration is Thursday, Aug. 4, from 11 a.m. to 6 p.m.
All families are required to provide the following information when registering students:
-Certified birth certificate (Applies only to students who are new to District 186 or students who don’t have a certified birth certificate on file.)
-Current physical examination (within one calendar year), current immunization and medical records (Applies only to students who are new to District 186 and students entering kindergarten)
-Current dental exam (Applies to students entering kindergarten and second grade).
-Current vision exam (Applies to students entering Illinois schools for the first time and all kindergarten students)
You will also be asked to provide several items to show proof of residency within District 186 schools. This can include the following:
-Driver's License/photo id/passport (with current address)
-Recent utility bill (with your name and address on it)
-Mortgage or lease agreement (with name and address) and most recent record of payment
-IDPA medical card
-Property tax bill
