Dear Parent:

In the fall of 2020 your child will be entering kindergarten, 6th, 9th or 12th grade. For entry into each of these grades, updated health information is required. Due to the closures associated with COVID-19, District 186 is aware appointments for physicals and immunizations may not be available at this time. Please contact your child’s physician regarding possible appointment dates prior to the start of the 2020-2021 school year.

School Health Requirements:

Kindergarten - Physical Exam and Updated Immunization Record including DTaP, Polio, MMR

and Varicella; Eye Exam; Dental Exam

6th grade - Physical Exam, Meningococcal Conjugate Vaccine on or after the students 11th

birthday, Tdap and the Hepatitis B series at appropriate intervals; Dental Exam

9th grade - Physical Exam; Dental Exam

12th grade - Meningococcal Conjugate Vaccine on or after the student’s 16th birthday
