Reminder! All students entering kindergarten, 6th, 9th grade, or new to the school are required to have a recent physical exam form and an up-to-date immunization record on file prior to enrollment. All students entering kindergarten and new students are required to show proof of an eye examination performed by an eye doctor prior to enrollment. All students in grades kindergarten, 2nd, and 6th are required to show proof of a dental examination by May 15th. The dental exam must be performed within the 18 months prior to May 15th.
If your child will be receiving medication during school hours next year, a newly acquired and completed authorization form along with a properly labeled bottle must accompany the medication. A written authorization from the parent and physician is required for all medication including, but not limited to, “as needed” medication such as inhalers, Epi-pens, and over-the-counter medications such as pain relievers. A self-administration parent and physician agreement is required for students who will be self-carrying inhalers/Epi-pens. Although, this practice is not recommended for students in grades K-2.
Students with health concerns including, but not limited to, asthma, diabetes, seizures, and severe allergic reactions must have a health plan on file with the school’s nurse/ office. If you would like this information to be shared with other staff and faculty such as the cafeteria staff, the PE teacher, and the classroom teacher, please note that information on the plan.
