Athletic Permit Card
District 186 now offers an online athletic permit card to fill out and use, eliminating the
need to complete a paper copy.
1. First, go to the district intranet page and log in to your parent account.

2. Once you are logged into your account, look at the left side of the computer screen and
locate the 'Athletics' section. Click the link.
3. Once you click “Athletics,
” it will take you to the screen below. Click on the blue link at
the top of the page that says “permit card.
”4. Once you click “Permit Card, a screen will appear with your child's name on it. Click on
Fill Out Form.
5. This is what the form will look like. Once you are done filling out the form, hit Submit at
the bottom of the page.
6. Once done, have your child log into their account so that they can sign off on the form,
and the form will be done. Your child MUST sign off on the form for it to be accepted and
marked completed.
