2023- 2024 School Health Requirements & walk In clinic options

2023 - 2024 School Health Requirements :

  • All Pre-K, Kindergarten, 6th grade, 9th grade students and students new to Illinois schools must provide a current physical exam and immunization record.
  • Students entering from another Illinois school must provide his/her most recent school physical and immunization record. 
  • All 6th grade students must show proof of having received all required vaccines including :
    • 1 dose of meningococcal conjugate vaccine must be received on or after the 11th birthday
    • 3 doses of Hepatitis B vaccine at approved intervals
    • 2 doses of varicella 
    • Tdap vaccine
  • All 12th grade students must provide proof of having received a dose of meningococcal conjugate vaccine on or after their 16th birthday.
  • Kindergarten, 2nd, 6th and 9th grade students are asked to provide proof of a dental exam. 
  • Kindergarten students and students new to Illinois schools need a vision exam by an eye doctor.
  • Physical exam and immunization information is due the first day of the 2023-2024 school year.

Physical exam and immunization information is due the first day of the 2023-2024 school year. Students failing to provide required health information will be excluded from school on September 1st (Balanced Calendar Schools) or October 1st (Regular Calendar Schools).

Mon-Fri, July 10 - 14th- Walk In Immunization Clinic at Sangamon Co. Department of Public Health, 8:30 a.m. - 11:30 a.m. , 1:00 p.m. - 3:00 p.m. 

Keep Kids In Schools | Back to School Physical  :

Wed, Aug 9th - Walk-in Clinic at Sangamon Co. Department of Public Health, 8:30-12:00. Partnered with SIU Department of Family Medicine.

Sat, Aug 12th - Walk-in Clinic at Sangamon Co. Department of Public Health, 8:30-12:00. Partnered with SIU Department of Pediatrics.

Sat, Sept 9th - Appointment only at Sangamon Co. Department of Public Health, 8:30-12. Partnered with Springfield Clinic.
