Parents and families: if you have a student who is entering Pre-K, Kindergarten, 6th grade and or 9th grade or have a child who is new to Illinois schools, you must provide a current physical exam and immunization record. Students entering from another Illinois school must provide his or her most recent immunization record.
All 12th grade students must provide proof of having received a dose of the meningococcal conjugate vaccine on or after their 16th birthday.
Please schedule your child’s school physical and immunization appointment as soon as possible. SIU Medicine, Central County Health Centers, HSHS Medical Group, Memorial Health and the Sangamon County Department of Public Health are taking appointments.
